Dennis Radman, MSW, RSW – Psychotherapy Practice
Effective Date: April 1, 2024
Effective Date: April 1, 2024
As a registered social worker (MSW, RSW) in private practice in Ontario, I am committed to protecting your privacy and ensuring the confidentiality of your personal information. This Privacy Policy explains how I collect, use, and safeguard your information in accordance with the Personal Health Information Protection Act (PHIPA) and the standards of the Ontario College of Social Workers and Social Service Workers (OCSWSSW).
Information I Collect
I collect personal information you provide when you engage my services, such as:
-
Contact Details: Name, phone number, email, and address to communicate with you and manage appointments.
-
Health Information: Details about your health, therapy goals, and progress (e.g., ABI recovery, chronic pain management) to provide psychotherapy and support.
-
Payment Information: Billing details (e.g., credit card or e-transfer info) to process fees, shared only with secure payment processors.
-
Digital Data: If we use telehealth or digital tools (e.g., behavioral tracking apps from my research), I may collect usage data to tailor your care, stored anonymously where possible.
I only collect what’s necessary for your therapy and won’t ask for more than needed.
How I Use Your Information
Your information is used to:
-
Provide psychotherapy services, including assessment, treatment planning, and progress tracking.
-
Communicate with you about appointments, updates, or therapy-related matters (e.g., via email or phone).
-
Process payments and issue receipts (e.g., for insurance).
-
Collaborate with other professionals (e.g., psychologists, SLPs) if you consent to information sharing for your care.
-
Comply with legal obligations (e.g., reporting imminent risk of harm as required by law).
How I Protect Your Information
-
Security: I store your records in a PHIPA-compliant, encrypted digital system (e.g., password-protected platforms). Any paper records are kept in a locked filing cabinet. Telehealth sessions use secure, encrypted platforms.
-
Access: Only I access your information unless you consent to sharing (e.g., with another professional) or law requires disclosure.
-
Retention: I keep your records for 10 years after our last contact, as per OCSWSSW guidelines, then securely destroy them (e.g., shredding paper, wiping digital files).
Sharing Your Information
I do not sell, trade, or share your personal information with third parties except:
-
With your written consent (e.g., for interdisciplinary care).
-
As required by law (e.g., child protection, court orders).
-
With secure payment processors for billing (they don’t retain your data beyond processing).
Your Rights
Under PHIPA, you have the right to:
-
Access your records (request a copy from me).
-
Request corrections to factual errors in your records.
-
Withdraw consent for certain uses of your information (e.g., sharing with others), though this won’t affect actions already taken.
-
File a complaint with me or the Information and Privacy Commissioner of Ontario (www.ipc.on.ca) if you have concerns.
To exercise these rights, contact me. I’ll respond within 30 days, or sooner if urgent.
Telehealth and Digital Tools
If we use virtual sessions or digital health tools, I’ll ensure they’re secure, but no online system is 100% risk-free. I’ll explain any additional risks (e.g., tech failures) and get your consent beforehand.
Changes to This Policy
I may update this policy as my practice evolves (e.g., new digital tools). Changes will be posted here with the new effective date. Check back periodically, especially if we’re working together long-term.
